Welcome to the Lifestyle Marketplace Fundraising Referral Program. Referrals are the greatest compliment that we receive. In appreciation, we would like to say "Thank You" by issuing a total of $1 to you for every sheet set that is sold by an organization that you have referred to us.

Simply, have the organization that you are referring go to our website, www.lifestylemarketplace.com, and click on the "Get Started Contact Form" link at the top of the page, under Fundraising. As they fill out their information, there is a space for "Referred By." This is for your information. Be sure that they put your name, phone number and/or email address. We need this information when we set up your "Referral" account.
Once we have received your "Referral" information, we will email you a W-9 Form to be filled out and returned to us. Once we have received your W-9 form, your account will be complete.
You will receive a copy of the invoice via email when it is sent to the organization that you have referred to us. This is so you can track how many sheet sets they have sold.
When this invoice has been paid, we will issue a check to you for $1 per sheet set sold.
